A longitudinal study from North Carolina State University, published in Frontiers in Veterinary Science, found that shorter front-limb stride length in aging dogs is associated with cognitive decline, even after accounting for age and pain. Eighty-eight senior and geriatric dogs were tracked over time, with gait measured on a standard indoor walkway and cognitive status assessed using the Canine Dementia Scale (CADES). The finding: a 10-point increase in cognitive impairment score corresponded to roughly a 1.2% reduction in thoracic limb stride length. Small in magnitude, but consistent, and independent of musculoskeletal pain.

What makes this clinically useful is not the effect size. It is the simplicity. No imaging, no specialized equipment. Just a five-meter walkway, a loose leash, and video to review.

Why the Front Legs, and Not the Back

The front-versus-back asymmetry in the study's findings is not arbitrary. Thoracic limbs play a primary role in braking and postural stabilization during movement, activities that require cortical processing, spatial awareness, and attention. "Thoracic limb movement is likely under more cortical influence than pelvic limbs and may be more sensitive to alterations in visual or spatial awareness than pelvic limb movement," said Dr. Natasha Olby, professor of neurology and the Dr. Kady M. Gjessing and Rhanna M. Davidson Distinguished Chair in Gerontology at NC State's College of Veterinary Medicine.

Pelvic limbs function primarily as propulsion motors and are more susceptible to coxofemoral osteoarthritis, which can obscure any cognitive signal in the data. The front limbs turned out to be the more reliable indicator.

The researchers are clear that stride length is not a standalone diagnostic. Its value is as an adjunctive, trackable marker: most useful when measured over time alongside owner questionnaires. "It could serve as a useful marker of an individual dog's overall health trajectory when it is monitored over time," Dr. Olby said. The method's reliability was strong, with intra-observer and interobserver intraclass correlation coefficients above 0.97.

When Getting to the Clinic Becomes the Barrier

Here is the part that does not always make it into research write-ups: for many senior dog owners, especially older adults, the logistics of the clinic are themselves a problem. The research and the programs emerging to serve this population point toward the same practical direction: care that comes to the patient.
